I authorize Foot Lab to use for promotional purposes photos or videos taken during the activities. I certify that the student is in good health and can practice a sport activity. I authorize Foot Lab to take any emergency measures necessary due to the child’s condition (unless otherwise specified in writing by me), and I agree to reimburse any medical expenses.
